Course Substitutions

Any student requesting or requiring a change in their Program II courses, should discuss the proposed change with their advisor. Afterward, you should complete the Course Substitution Form below and sent to your advisor for final approval. The change will be official once your advisor signs off on the substitution and sends the approval to the Program II Director.

Note: Changes must be made before the first day of class for the original course.

How to Complete the Course Substitution Form

Students' Responsibility
  1. Open the Word form below
  2. Complete the top portion (up to the section that the advisor must complete)
  3. Save the Form (click File->Save As). You can Save the form in your Documents or on your desktop
  4. Open your Email and create a new message to your Advisor
  5. Then click Attach File, locate the Form you saved and attach it
  6. Send the email with form to your advisor
Advisors' Responsibility
  1. Open the email attachment (form)
  2. Answer the three yes/no questions regarding the proposed substitution
  3. Save the Form (click File->Save As). You can Save the form in your Documents or on your desktop
  4. Open your Email and create a new message to the Program II Director (rachael.murphey-brown@duke.edu)
  5. Then click Attach File, locate the Form you saved and attach it
  6. Send the email
